Does it slip out in a particular gear or all gears?

The shifting fork can be bent and not pushing the gear all the way into place.
The shifting dogs are tapered.

I have seen many riders kick the shift lever or downshift without revving the engine.
I have seen riders down shift through all the gears without revving the engine even once.
