Crunchy chain. Yikes.

My chain seems to have developed a worrying symptom. As it rotates on the rear sprocket, it makes a crouching/snapping noise. It seems like it happens in a different spot on the chain each time, though close. The chain issue is caused by the chain snapping out of alignment with the sprocket, as if the sprocket had a worn spot or was bent. This is strange because the wheel is straight, as is the sprocket. Before I get a new chain/sprocket set, is there anything I could be  missing?
What am I doing and why am I doing it? Those are excellent questions.

Its kinking, to clean it up , lube each link where the plates slide against each other..one side, then the other all from inside the chain,,,,ie ABOVE the lower run..then a drip on each roller..
 Rotate it a few times or go for short easy ride.  Once its lubed again..You can actually see what you have left .

Check the countershaft sprocket also while your checking stuff out,they are smaller and turn more and get worn out. How old is the chain and is it lubed well ? Any other mods on the bike like wheels etc over the years ?

Unless the drive set is fairly new, I would tend to rely on your initial assessment and self-recommendation.

Any time these components start making (crunchy, popping, grinding, whining, rhythmic) expensive noises, I replace them.

Breaking a chain at 50mph on the way home from a midnight shift and popping a hole in the case has affected my judgement in these matters.  YMMV.
