CB550f front brake issues

So, while I am waiting for some parts, its time to get the front brake done. 

New pads, stainless top hose, the piston looks great, rubber seal is complete, and new pin on dead pad.  spring and bolt are locked into position.

Issue is when pushing it, it catches and squeakes.  Does this when riding too. 

I have done the adjustment, had a credit card worth of space on both sides.  wheel spun freely.  this changed after first ride.  Will not stay in adjustment. 

any ides?  what needs to be done to make this work right?  Warped rotor?

Make sure the arm can float & follow the rotor, anti squeal grease behind the pads, rough up the rotor with a coarse sandpaper & finally, ensure the piston retracts when the lever is released.
