That is a domed steel "cap", that fits into the end of the piston. The domed end contacts the back side of the pad.
Yes, you just screw it in, I used screw from old pad in a new pad at least once when the new one came without it.The screw itself has a big head - the size of the groove - it is not just regular Home Depot piece.
I don't really see the point of the screw but I am sure as with 99.9% of Honda stuff that there's a reason for it.
