CB750 Clutch lever get loose

Have you also had the problem with sudden no working clutch due to the lever outside clutch has fallen off or almost?
First time the nut came off and arm wobbled of the position so it did not push clutch plate inwards. I glued the nut so next time the adj screw unscrewed itself until clucth did not work.

The clutch arm will cause pressure on nut so it will unscrew itself when using the clutch. Is it grease missing or nut too tightened?
