Looks pretty "groovy" and guessing a replacement is needed.

I had to order one side of the tensioner slider and tensioner blade from Honda and one from DSS if I recall correctly.
might be confusing it with the cx500 as I ordered them at the same time.

That is fairly normal wear as long as the rubber is not cracked at the ends and the part in between the frooves is not a lot lower than outside.
The sideplates of the chain wear into the rubber till the rollers touch
Semi Geriatric ex-Honda mechanic and MOT tester (UK version of annual inspection). Garage full of "projects" mostly 500/4 from pre 73 (no road tax in UK).

Remember "Its always in the last place you look" COURSE IT IS YOU STOP LOOKIN THEN!

It truly is best to get new reproduction tensioner and guide for these motors. 
They'll run quieter, and ya won't find chunks of it in the oil pan or lodged against the cam sprocket like I did  :o
