There could be blocked passages in the block or head, this would / could happen if the coolant was not changed over many years it gets acidic. If the radiator was blocked up the same thing can happen in some small passages in the head.
Running in the garage doesn't put a load on the engine, you can get by with a marginal cooling system under low or no load.
The other thing that probably has been checked is the thermostat, if not pull in out, you can check if it opens in a pan of hot water on the stove and a thermometer ( it should open by 190 degrees). You can pull it out and run the motor without it but don't leave it that way for weeks, it will be running rich. If it's still hot without the thermostat and a new radiator, then flush the living daylights out of the motor with a flushing solution from any auto parts store, Prestone makes a good one, it takes a few flushes sometimes.
