Below is my letter to management as my manager required. I'm fine but a little stiff feeling, the bike is fine (Honda 919) with the exception of a scuffed clutch cover and bent front brake lever. Parking lots suck.

I wanted to let you know that today while departing the parking lot on my motorcycle I had to lay my bike down and jump off in order to avoid collision with an oncoming car occupying my lane.
I left the motorcycle parking area at the rear of the building and turned right to leave the parking area. As I neared the turn for the outermost lane, which runs along the side of the building, I saw a SUV approaching in my lane. They were on my side of the road as they turned left towards me and proceeded directly at me. At about 5 mph I turned right, braked, and jumped off my motorcycle to avoid a direct hit.
The SUV stopped next to me. Then moved on just a little so they could get out of the vehicle. My bike tires were less than 4 inches from their tires with my bike laying down on its right side. If I wouldn't have ditched my bike and jumped off I would have been hit head on.
The driver of the car was XXX XXXXXX, and is the friend of the wife of an RHR employee. I did get her insurance policy information and personal information.
