Bill of Sale question

I am shipping a bike from the US to UK and have to provide a 'Bill of Sale' along with the Title.
Does it have to be in a semi-legal form as I have seen in online templates or is a simple receipt ok in the form of;
'I Mr Seller accept $xxx from My Name for the sale of motorcycle Name/Model, VIN xxxxx Signed'

I would use a legal, notarised form to sign and send back a copy to the seller for his records.
Might as well do it proper if you're going to do it.

Notarized means that it first has to be a legal document of some sort and it basically gives you a witness that says you got the money or paid the money or what have you. You get a document notarized at a notary public, most banks have a certified notary at them.

I always thought that a notary just 'testified' that they witnessed you sign your signature and that you provided some identity that you were you, or that they personally knew you.  They do NOT swear as to the validity of any statement made on the paper.

How could they?  How could they KNOW that you got the money, or whatever it says.

They can't, so the statement always says something to the effect of "sworn before me this xxx day of yyyy".

So, there are certainly templates online, but I'd just go with your own handwritten bill of sale because the vehicle title is the main thing.  But, a bill of sale should include:

Seller's name (printed),
Seller's signature,
buyer's name (printed),
date,
purchase price,
Vehicle year/make/model and VIN #. 

And some statement to the effect that this is for "paid in full"

Thanks Kirk that is more or less what I was thinking. I currently have just an eBay end of sale print out, but getting a written one will not be a problem. I think the Bill of Sale is needed to show the UK Customs the value for Duty payable reasons. I understand the Title is a vauable piece of paper unlike in the UK where duplicate Registration Documents can be easily obtained.

You are correct. When I Notarize a document you are making a sworn statement and if that statement is false you are guilty of perjury. I Notarize documents in Japanese all the time, I do not read Japanese.
