... you had the front of you car (front WD) up on ramps and you forgot to release the E-brake when you tried to back down?

My car wouldn't move, as I always place wheel chucks behind the rear wheels.

Once had a Ford Aerostar (Stick, thank you) that my wife was warming up in the driveway. Didn't check the emergency brake. Walked away - van rolled down the drive backward, across the street, and slammed into a neighbor's garage. Nobody hurt - but sh*t - that was embrassing.

BE CAREFUL.

I borrowed a fiend of mines truck once, the only problem was he forgot to tell me the shift linkage for the trans was severely worn and had a habit of jumping into reverse from park. I started it up then hopped out to nab the keys from my car and as I was walking back to his truck I see it's going down the road in reverse with nobody in it, I'm not a sprinter but it's amazing how fast you can run when motivated. By the time I caught up to it and got it stopped it had already taken out 2 mail boxes and gone across a neighbors lawn heading straight for the side of another neighbors house, I then had to deal with a thoroughly pissed off neighbor demanding to know why the truck was setting in the middle of his lawn. The neighbor didn't believe my story at first but fortunately his young son saw the whole thing and said "Dad, the truck really was going down the road with nobody in it".

As an FYI, some later model cars with aftermarket security systems can and will,in
some instances, start and run on their own WITHOUT turning the key on. It can happen
if the system is not powered down correctly. Most have a precautionary label
affixed on the core support or somewhere visible in the engine compartment. The ones I have had experiences with state that the brake pedal should be applied on for at least x-amount of seconds AFTER battery is disconnected, to prevent possibility of engine starting on its own after power is restored. It will scare the $h!t outta you if it happens, and please don't ask how I know this :P

my 33 ford truck with 66 -model C4 and radical HiPo289 will jump in reverse and head down the road ,  chased it a few times and now it only idles unattended in natural and E Brake on , a lot of old Fords were known to do this.

   I just love stories where guys are running after the vehicle, and are lucky enough to stop it without getting run over.
 My story doesn't have a transmission popping gear problem, or a faulty E brake issue, just an operator oversight.
I'm putting up fence for a customer in CT when my fence stretcher fails, I'm in the middle of suburbia and it's a long ride anywhere to get another one. So I figure , I'm almost finished, why not put the 350 dually on the loose end of this fence, give it a tug  and attach the fence to the posts?  I was on the downside of a slight incline so there would always be pressure on the fencing. What could possibly go wrong, right?
  So this was working like a charm , I had the run all stretched attached and I was ready to go after I cut the excess off.
Nice job ,I was thinking as I cut the fence , the weight of the truck kept the fencing nice and tight so I could make accurate cuts with ease...finally the last cut. Snip. That's when it occurred to me...  as the truck started to move down the incline, that I didn't use the E brake or have it in gear...so smooth and silent  she slipped away.
  I was catching up to it by the 1/2 acre, was up on the aluminum step trying to get the door open, by the end of the acre when I realized I'm running out of field and the woods are coming up quick.
  Got it stopped before we hit one of the larger pines on the property. That was embarrassing! Lucky the homeowner wasn't there to see that! I was getting all my tools back on the truck when she shows up and so does her neighbors and they saw the whole thing ...and hadn't had a good laugh like that in a while.
   I'd like to tell you that I don't do stupid things like that anymore.....but that just isn't true.
A few Honda 500's, a few Honda 550's, a few Honda 650's, '72 cb 450, a couple 500/550/650 hybrids, and 2001 750. 
  550 Snowbike -Somebody had to do it.

  There is a Drag Racer I know that had his dragster on jack stands and a floor jack under the rear axle. He was breaking in a new engine and warming it up in gear, to check the trans for leaks.  He got out, went for a tool and when he stepped away the floor jack bled down. The car got traction, left the shop and covered quite a bit of farm field going out of sight. He jumped on a 4 wheeler and chased it down, finding it had crossed two fence lines and ended up partly submerged in a creek.
  At the end of year banquet someone gave him a snorkel and swim fins. They also named him the U boat commander. He didn't take it well.

I had a jeep in high school.  4.0ho straight six and the peugot 5spd transmission with that lovely transfer case i cant remember the name of.  Aaaaahhhh youth, man that cherokee saw A LOT of action with all that cargo space.
Anyway, friday night football, I played sax in the marching band, bunch of the jocks had jeeps and muddy trucks.  They all parked their rigs flexed and posed against this earthen retainer wall mound. 
Not to be upstaged, I rolled my stock cherokee xj up on the mound with the rest of the 4x4's.  I had this thing practically balanced on two wheels, full suspension articulation.  It's quite impressive how much the suspension on a stock jeep can travel. 
So I shut down, get out, grab my horn and do musical interludes for the next 2 hours.  Game over, back to the parking spot, and my jeep is flat on the ground.
Didn't set the parking brake, and the transmission held just enough resistance to keep the suv from wreaking havoc in the parking lot. 
Its probably all for the best that the jeep ended up firmly on four wheels, if that group of jocks had seen this band nerds' jeep set so precariously, they may have tried to tip it!
