Clunk from transmission under load

If I drive normally and accelerate in 1st 2nd and 3rd gear there is a clunk noise a momentary loss of power and some transmission slipping.  Feels like gears are jumping.  Sometime if makes a rattle after the clunk.  If I ride very slowly it hardly happens.

Any ideas? 

 Also,  the clutch is pretty worn out, and needs to be replaced.

What do your sprockets look like? If severely worn, the links of the chain can jump over the worn teeth..

I hade a chain whereappr.  1/3 of all the rollers were gone... similar problem until the chain jumped off in low speed, 30mph. Before that drove like a racer champ 100-110 mph on a small winding road....
