Rear Retainer backed out

Guys I'm at a loss.  Recently rebuilt my rear hub (76 cb750F).  When I went to check out my handy work I put it in first gear and went about 100 feet.  Got off the bike only to notice that the rear bearing retainer had backed almost all the way out and the collar had crushed through the retainer. The retainer was now pressed hard against the rear brake caliper.   I have a feeling I may have over tightened my rear axle nut and this may have caused the collar to lock into the retainer and spin it out.  Any thoughts?  Just put in a new order for a new retainer so I'm dead in the water. 

Guess I should be thankfull I didn't get too far from the house. 

Suggestions appreciated.

DId you re stake the retainer when you installed it.

Did restake it.  However, perhaps I didn't put big enough divots in it.  Just to confirm the wider part of the collar should be touching the rear brake caliper? or should their be any space between the rear brake calipar and collar?  I am wondering if I just over tightened the rear axle and caused to much pressure.

With the rear wheel properly assembled, correct part, spacers, bearings, etc. that should be impossible.

Actually ranger is right. I would check a parts fiche or download a manual and check your assembly against the drawing.

Agree with CR and BR. It is not possible to overtighten the axle nut. If anything, most people do not tighten it enough. Should be 75ftlbs.

Can't help you with your problem other than to steer you away from that idea.

Checked and double checked and all was put back together correctly.  Guess I'm SOL on this one.  I'll restake it a little betther next time and see what that does.

1+ to CR, BR, and MC: something is wrong/missing. Did you leave out the inner bearing spacer sleeve? Pics might help...

This is my second time dealing with the rear retainer coming out. 
I'm pretty sure the first time the bearing failed first.  This time the new (~5k miles) bearing is also failed, but I do think the bearing retainer backed out first. 

After the first failure, I had to grind away 2.5-3 turns of the thread from the hub, so there is less engagement.  I did stake it, but was probably apprehensive about pushing in too hard into the reduced hub thread.

Has anyone found it necessary to add a thread locker to this assembly?

Is there a better (than the scanned third hand photocopy I was using) exploded view of the rear assembly somewhere, to be sure I've got all the parts and in the right order?  I was sure that I had an all steel-on-steel clamp of the bearings and spacer tube, but I'd feel better running through the mountains after double checking the BOM.

2 things:

 - Forgot to mention that this is a '78 750 F2 with rear disc.

 - Never mind anyway.  I see now that I am missing a piece, the spacer between the two bearings on the sprocket side.  So the bolt clamp force was putting side loads on both the outer bearings.  It probably was a bearing failure again.

Now we'll see about getting the larger sprocket side bearing off without molesting the smaller inside bearing.

Already got a whole spare rear axle, with all the bits on it, on order off ebay for $30.  :)

...which is a bit less than the new retainer, seal, two bearings which will also be on order.

On to the O2 sensors on the pickup!

Those will certainly make bike parts seem cheap! ;)
